SoftDental — Dr. Minh Nguyen, DDS, PA

You wake up with a headache. Your jaw aches by noon. You hear a clicking or popping sound every time you open your mouth wide. Your ears ring for no reason. Your neck is stiff. You are exhausted — and no one seems to connect all these symptoms to the same source. That source is often your temporomandibular joint — and it is more treatable than most patients realize.

What Is the TMJ?

Your temporomandibular joint — the TMJ — is one of the most complex joints in the human body. It is the hinge that connects your lower jaw (mandible) to the base of your skull, just in front of each ear. Unlike most joints, it moves in two ways simultaneously: hinging open like a door and sliding forward and backward. This makes it capable of remarkable movement — and also vulnerable to dysfunction.

The TMJ contains a small cartilage disc that cushions the joint. Muscles, ligaments, and tendons surround it. When any part of this system is out of balance — whether from grinding, bite misalignment, jaw injury, arthritis, or stress — the entire system can break down. Temporomandibular disorders (TMD) encompass a range of problems involving the masticatory muscles, the TMJ, and associated structures, with common symptoms including pain, restricted movement, muscle tenderness, and intermittent joint sounds.

🦴 What is actually happening inside the joint In a healthy TMJ, the cartilage disc sits between the condyle (the ball of the lower jaw) and the socket of the skull. When the disc slips forward — a condition called anterior disc displacement — the joint clicks or pops as the condyle moves over it. Over time, the disc can become stuck in a displaced position, causing the jaw to lock or limit opening. The bone surfaces, without cushioning, can begin to wear against each other — leading to arthritis and permanent bone changes.

How TMJ Disorder Affects Your Daily Life

TMJ disorder is not just a dental problem. Temporomandibular disorders affect fundamental functions such as chewing, speaking, swallowing, and facial expressions, significantly influencing overall quality of life. The impacts reach far beyond the jaw — affecting work, sleep, relationships, and mental health.

🦷

Jaw Pain & Tenderness

Aching, throbbing pain in the jaw — often worse in the morning if you grind at night. Tenderness when touching the jaw muscles.

🤯

Headaches & Migraines

Chronic tension headaches — especially at the temples, behind the eyes, and at the base of the skull — that do not respond to usual remedies.

👂

Ear Pain & Ringing (Tinnitus)

The TMJ sits directly in front of the ear canal. Inflammation and tension in the joint can cause ear pain, pressure, and ringing that has nothing to do with your ears.

🔊

Clicking, Popping, or Grating

Sound and sensation when you open your mouth, chew, or yawn — often the first noticed sign of disc displacement in the joint.

Limited Jaw Opening

Difficulty opening your mouth fully — or a jaw that locks open or closed. Even minor limitation affects eating, speaking, and dental care.

🦴

Neck & Shoulder Pain

Jaw muscle tension radiates into the neck, upper back, and shoulders. Many patients seek physiotherapy for pain that is actually driven by the TMJ.

😴

Disrupted Sleep

Grinding or clenching during sleep — bruxism — both causes and worsens TMJ disorder. Poor sleep worsens pain perception, creating a damaging cycle.

🧠

Anxiety & Depression

Clinical research confirms: anxiety, depression, and stress play a crucial role in TMD onset and progression. Chronic pain changes how the brain processes information and experience.

⚠️ The cycle that makes TMJ disorder worse without treatment Stress → clenching and grinding at night → jaw muscle fatigue and joint compression → pain → disrupted sleep → more stress → more grinding. Without intervention, this cycle accelerates. The joint that was mildly clicking at 35 may be arthritic and locked at 50. Early diagnosis and treatment matter.

How Dr. Nguyen Diagnoses TMJ Disorder with the Anatomage 3D CBCT

Most TMJ diagnoses in general dentistry rely on clinical examination, patient history, and a standard 2D panoramic X-ray. This gives a general overview — but it misses critical detail. Due to relatively low radiation doses and excellent spatial resolution, cone-beam computed tomography (CBCT) is being utilized more frequently in dental-maxillofacial imaging for the diagnosis and treatment of TMJ disorders. Conventional TMJ imaging has significant limitations due to its two-dimensional view and adjacent anatomical superimposition.

At SoftDental, Dr. Nguyen uses the Anatomage Cone Beam CT system — the same advanced 3D imaging platform used in medical schools and major hospitals — to image both TMJs in full three-dimensional detail before formulating any treatment plan.

What the Anatomage 3D CBCT Shows in a TMJ Evaluation
2D Panoramic View of TMJ Flat shadow — limited detail Anatomage 3D CBCT — TMJ Detail Condyle, joint space, and bone changes clearly visible Joint surfaces overlap Bone changes not clearly visible ? ? Left condyle ✓ Healthy shape, disc in place Right condyle ⚠ Bone erosion detected Joint space Articular disc Erosion

Left: A 2D panoramic X-ray gives a blurry, superimposed view of both TMJs — critical bone changes and disc position are nearly impossible to evaluate. Right: The Anatomage 3D CBCT gives Dr. Nguyen a precise cross-sectional view of each condyle individually, measuring joint space, detecting bone erosion or arthritis, and confirming disc position.

With the Anatomage system, Dr. Nguyen can evaluate both condyles independently, measure the joint space bilaterally, identify early-stage bone erosion or remodeling, detect flattening, osteophytes (bone spurs), or subchondral sclerosis — changes that indicate how far the TMJ condition has progressed and what treatment approach is appropriate. This level of diagnostic detail is the difference between treating a symptom and treating the actual cause.

🔬 What the 3D scan specifically shows for TMJ patients Precise shape and symmetry of both condyles · Measurement of joint space on both sides (asymmetry suggests disc displacement) · Any erosion, flattening, or spurring of the condyle head · Degree of mouth opening movement captured · Positioning of the condyle within the fossa · Signs of osteoarthritis in the joint · Relationship to nearby structures including the ear canal

The Custom Occlusal Guard: Made with the iTero Digital Scanner

Once the diagnosis is complete, the most common first-line treatment for TMJ disorder is a custom-fitted occlusal splint — often called a night guard or bite guard. This is an appliance worn over the upper or lower teeth that achieves several things simultaneously: it stops grinding from damaging your teeth and joints, it holds your jaw in a relaxed, slightly forward position that decompresses the TMJ, and it breaks the clenching cycle that perpetuates jaw muscle tension.

The key word is custom. An over-the-counter night guard from a pharmacy is a one-size-fits-all piece of plastic that can actually worsen TMJ symptoms by positioning the jaw incorrectly. A properly fabricated occlusal splint is designed from an exact model of your teeth and your specific jaw anatomy.

At SoftDental, Dr. Nguyen uses the iTero digital scanner to capture a precise 3D model of your teeth in under 60 seconds — no messy impression trays, no putty. The digital scan is sent directly to the dental laboratory with Dr. Nguyen's specific prescription for the type, thickness, and jaw position for your appliance. The result is an appliance that fits your teeth with a precision that a hand-poured impression simply cannot match.

Most patients with TMJ disorder have been in pain for years before they come to me. They have seen ear doctors for tinnitus, neurologists for headaches, and physical therapists for neck pain. When the jaw is the source, treating everything else helps — but does not resolve it. The 3D scan shows us exactly what is happening in the joint. That is where the real answer starts.
