When patients have lost most or all of their teeth, they often hear about All-on-4 as the answer. It is heavily marketed, and it can be a good option for the right patient. But there is another approach that Dr. Nguyen often recommends — one that is easier to clean, carries a lower risk of infection, and lets the patient manage their own oral health far more effectively: the mini implant-supported overdenture.

Why Dr. Nguyen Often Prefers Mini Implants
Dr. Nguyen's recommendation is always based on what is best for that specific patient — not what generates the highest revenue or what is most heavily marketed. After treating many full-arch implant cases, his honest assessment of mini implant overdentures is that for the right patient, they offer three meaningful advantages that All-on-4 simply cannot match.
You Can Actually Clean It Properly
When you can remove the denture, you can clean every surface of it — and every surface of the gum and implant area underneath it. Thorough cleaning reduces bacterial accumulation dramatically. With a fixed All-on-4, you are cleaning in your mouth with specialized brushes and water flossers — and areas under the bridge accumulate plaque that is far harder to fully remove.
Lower Risk of Peri-Implantitis
Because the mini implant overdenture can be fully removed for cleaning, patients achieve better biofilm control around the implant posts. Less plaque means a significantly lower risk of peri-implantitis developing. For a fixed bridge like All-on-4, plaque under the bridge is a chronic challenge — and peri-implantitis in an All-on-4 case is one of the most difficult dental complications to treat.
Easier to Monitor and Treat
If a problem does develop — tissue inflammation, a loose implant, any concern — Dr. Nguyen can simply have the patient remove the denture, examine the implant area directly, and treat it straightforwardly. With a fixed All-on-4, accessing the implant area for treatment requires removing a permanently cemented bridge — a complex procedure that may damage the restoration.
Works for More Patients
Full-size implants for All-on-4 require adequate bone volume and density. Mini implants, being narrower (1.8–3mm vs 3.5–5mm), can be placed in patients with less available bone — including patients who have experienced significant bone resorption after years of wearing traditional dentures. Mini implant-retained overdentures have been shown to provide benefits including reduced bone resorption and enhanced esthetics.
Research Shows Patient Satisfaction
Clinical studies show mini-implant-supported overdentures had better patient satisfaction levels compared to standard diameter implant overdentures. Patients appreciate the security compared to traditional loose dentures, while maintaining the ability to clean properly.
Lower Total Investment in Most Cases
Mini implant overdentures typically cost less than All-on-4 — both in the initial procedure and in long-term maintenance. If a denture needs to be replaced after years of use, this is straightforward with an overdenture system. Replacing an All-on-4 arch is a major procedure.

When All-on-4 Is Still the Right Answer
Dr. Nguyen is not saying All-on-4 is wrong. For some patients, it is genuinely the best option. A fixed, permanent prosthesis that feels like natural teeth is a meaningful quality-of-life upgrade — and for patients who are highly motivated hygiene patients with adequate bone and no history of gum disease, All-on-4 success rates are excellent.
I've seen All-on-4 cases fail because patients couldn't effectively clean under a fixed bridge. Peri-implantitis in a fixed full-arch case is one of the most difficult situations in dentistry. When I can recommend a system where the patient removes the prosthesis every night and cleans everything properly — I know that implant has a much better chance of lasting 20 years. That is what guides my recommendation.
